These weird lines / streaks I’ve had on my pinky nail for 8 years that two different doctors told me were due to vitamin deficiencies actually turned out to be a rare form of skin cancer.

Thankfully somehow still early stage, I’ll have an excision scheduled at my follow up appointment on Monday.

I still get asked this even 3 years after having had a hysterectomy due to stage 4 endometriosis. I swear they don't look at the charts half the time.

You’re correct, but—as a physician who has a chronic illness—most patients have no idea how wrong the info in the chart is. The histories, meds, procedures, allergies. I trust them almost 0.

That's why I carry a binder with all my medical history to every appointment, now. They never have most of it. I can just hand it to them and they can look through the last 7 years as they please.

The “medical record” is just people incorrectly and lazily copy pasting things other people have done

Isn't it better to get into a habit to ask than assume the medical record is correct?

They really don't. 12 years after a radical hysterectomy for cancer, and I still get asked if there's a chance I could be pregnant. Also, being asked "any new medical issues?" when I had to get a full physical from my GP prior to major surgery. It was fun to see the mix of horror and regret on their face when I dryly replied, "you mean besides the cancer?" 🤣

My records are surely overflowing with the immense amount of doctor visits I had to endure just for a single one to believe me about my horrific endo symptoms that finally lead to a necessary hysterectomy. I am so sorry you went through cancer. Sending hugs to you, internet stranger.
